PET-CT showed hypermetabolic primary lesion (1.2 cm, SUVmax 11.5) with uptake in the lower right hilar (1.0 cm, SUVmax 8.8), right hilar (1.8 cm, SUVmax 7.4), and anterior lower paratracheal (1.0 cm, SUVmax 8.0) lymph nodes. No distant metastases. EBUS biopsy showed adenocarcinoma in station 10R and 4R; malignant cells in 4L; atypical cells in station 7 (malignancy not excluded). The multidisciplinary tumor board recommended curative-intent chemoradiation followed by durvalumab. Radiotherapy was planned for 60 Gy in 30 daily fractions. Faculty:Dr. Houda BahigWorkshop Schedule:IASLC World Conference on Lung Cancer (WCLC) - Sunday, September 7, 2025Case Description:A 66-year-old male was diagnosed with biopsy-confirmed squamous cell carcinoma of the left upper lobe. At initial presentation, the tumor measured 6.5 cm and was staged as T3N2. Endobronchial ultrasound confirmed involvement of mediastinal and hilar lymph nodes, specifically at levels 4L and 11L. Baseline pulmonary function tests showed an FEV₁ of 1.5 L (50% predicted) and a DLCO of 40%, making him a borderline surgical candidate. The patient underwent three cycles of chemoimmunotherapy, achieving an excellent radiologic response. A post-treatment PET/CT demonstrated no FDG-avid lymphadenopathy and a residual PET avid mass in the left upper lobe. Repeat VO₂ max testing following systemic therapy showed mild deterioration of pulmonary function test and VO₂ max testing was <10 ml/kg/min), making him ineligible for surgical resection. The patient is sent for consideration of radiotherapy. The final pathology confirmed pT1c N2 M0.
