"Atelier de simulation de planification avec EduCase" qui a été organisé dans le cadre du congrès annuel de l'Association des radio-oncologues du Québec (AROQ)

Un homme de 56 ans est diagnostiqué avec un adénocarcinome du rectum moyen. La résonance magnétique du pelvis décrit une masse à 9.5 cm de la marge anale et qui mesure 4.7 cm en longueur. On note quelques ganglions mésorectaux aussi. Le stade clinique par IRM est cT3 N+. Le bilan d’extension ne démontre aucune métastase à distance. Un traitement de chimioradiothérapie (45 Gy en 25 fractions suivi par un boost tumoral) est proposé. Une résection mésorectale totale (TME) est prévue par la suite.

Le patient est convoqué en radiothérapie pour un scanner de planification. La résonance est fusionnée.

Tâches
1. Tracez la tumeur rectale (GTV-T)
2. Tracez la limite supérieur et la limite inférieur du CTV 45/25
3. Tracez le CTV 45/25 sur coupe axiale Z : 3.00 cm (image slice 80)

A 56 year old gentleman has been diagnosed with an adenocarcinoma of the mid rectum. The pelvic MRI describes a mass at 9.5 cm from the anal verge and which measure 4.7 cm in length. Several mesorectal nodes are also appreciated. By MRI the patient is staged as cT3 N+. The staging workup does not show and distant metastasis. Pelvic chemoradiotherapy (45 Gy in 25 fractions followed by a tumour boost) is proposed. A TME resection is planned afterwards. The patient undergoes a planning CT simulation scan. The pelvis MRI is fused.
